Combine three signals: questions that require a hold, cases returned for correction, and topics that produce repeat contacts. Look for patterns by queue and request type rather than treating every uncertain call as an individual performance issue.

Write the gap as a customer question and the decision the representative must make. Then provide a short approved answer, an exception example, and the boundary that sends the case to a specialist.

Measure whether the fix changes outcomes over the next review cycle. If accuracy improves but transfers rise, the answer may be clear while the permission boundary remains too narrow.
